Usually for LJ skins, I just edit the existing CSS code template. For graphics, I make it photoshop and upload it somewhere (like photobucket). It takes a while to get the process right but eventually you'll understand it.
Sometimes if you want a premade skin, check out http://community.livejournal.com/premade_ljs/. They have good skins and even a tutorial how to cut and paste the code. I learnt from that, and after studying codes, I understood how to tweak it here and there.
Its mostly trial and error and I'm not sure if my skins work in IE cause I use Firefox and only Firefox. Eep.
Maybe oneday I shall make a LJ skin tutorial 101? Lol.
I actually don't mind making skins for people, though it'll take me a while to get it done.
